At BABY STARS, we respect your privacy.

Dear Parents,

To ensure that you and your baby enjoy an incredible experience in a safe, clean environment, please observe the following:

  • Leave all strollers outside--next to the building.
  • Restrict food to the front reception area only. Adults must clean-up after their child.
  • Prior to classes or activities, please sign in and make any necessary payments.
  • Management reserves the right to charge for additional children to participate in activities.
  • No shoes permitted in the carpet area. Everyone must wear socks. If you forget to bring your socks, you may purchase a pair from the receptionist.
  • Dress your child in practical, comfortable, clothing
  • Change diapers only in the restroom.
  • Inform us of any medical condition and allergies.
  • Do not bring your child to Baby Stars if he or she is sick.
  • Cellular phones and video cameras are not permitted in class.

The following rules and regulations apply to all visitors to or users of the BABY STARS Web Site, the home page of which is at www.baby-stars.com.

By using this Web site, you agree to be bound by these rules and regulations, as they may be modified by BABY STARS at any time and posted on this Web site.

In the event of a violation of these rules and regulations, BABY STARS reserves the right to seek all remedies available by law and in equity.

 

I hereby enroll child/participant in the Baby Stars program referred to hereinabove.

I represent that I am the parent or legal guardian of the child/participant named above and that I have authority to enter the child/participant into this program. As the parent or guardian, of the child/participant named above, I release from responsibility and agree to indemnify and hold harmless, Baby Stars, its officers, directors, employees, independent contractors, and volunteers from any illness or injury to me, my child or family members. I hereby give authorization to Baby Stars to secure proper emergency services, and medical treatment from a hospital or physician for my child in the event on an emergency, with the costs to be paid by me.

Payment is due at the time of service. Payment arrangement must be made in advance of class. I hereby agree that I am fully responsible for payment. In the event that Baby Stars commences collection action to recover past due fees or charges, etc. for any program in which my child is enrolled, Baby Stars shall be entitled to reasonable attorney's fees and costs.

Cancellations and Schedule Changes:

  • Baby Stars reserves the right to cancel classes with insufficient enrollments and to modify class schedules as necessary.
  • Any class cancelled by Baby Stars will be rescheduled at no cost.

Mommy and Me classes are restricted to one parent and one child. All children must be accompanied by an adult. There may be extra fees for additional children attending the class that are payable at the time of the class. Please refer to our fee schedule for amounts.

I hereby give permission to Baby Stars and all persons acting with its permission, to obtain, use, copyright, and/or publish photographic portraits or pictures of the child/participant and accompanying adult, whether such pictures are still, moving, single or multiple, and authorize use of their name(s).

I HAVE CAREFULLY READ, UNDERSTAND AND, VOLUNTARILY ACCEPT THE TERMS & CONDITIONS OF THIS AGREEMENT.
